Instruction for completing the sample application form

Laboratory number and date sample received	To be completed by sample reception
Name and address details	Registration number: Enter the number under which you are registered with the NAK. If you do not yet have a number, our customer relations team will create one for you Applicant: Enter your company name here Address: Enter your address as completely as possible
Seed lot number	Enter the complete seed lot number including the country code A seed lot may consist of a unique lot or a combination of different lots
Species	Enter the species name of the submitted sample (Dutch or scientific name)
Variety	Enter the variety name of the submitted sample
Category	Fodder crops: PB, PB1, PB2, BZ (basic seed), CZ (certified seed) or HZ (commercial seed) Flax: PB, PB1, PB2, SE (super elite seed), E (elite seed), C1, C2 or C3 (certified seed, first, second or third generation) Cereals and other species: PB, PB1, PB2, BZ (basic seed), C1, C2 (certified seed, first or second generation) or HZ (commercial seed) PB stands for pre-basic seed, the generation closest to the original variety. Each following category (PB1, PB2, C1, C2, C3) is one generation further removed from the pre-basic seed.
Packaged in	Cross out what does not apply
Total weight of the seed lot	Enter the total weight of the seed lot
Number of containers	Enter the number of packaging units and the weight per packaging unit For different types of packaging, use the space above and below the dotted line
Country of origin	Enter the country code with two letters (e.g. NL, BE, DE, PL). If there are multiple countries of origin, list all country codes.

Label type	Indicate which label type is attached to the seed lot (e.g. EU-label, OECD-label)
Disinfected sample	Indicate whether the sample has been disinfected State the disinfectant used in the remarks and on the sample bag
Lot sealed	Indicate if the seed lot is sealed after sampling
Analysis	Germination: – NAK method: analysis of 300 seeds – ISTA method: analysis of 400 seeds Choose the analyses on which you want the sample to be examined
ISTA certificates	Indicate how many ISTA certificates you require and in which language
Remarks	Use this field to record any other remarks about the sample, such as the name of the disinfectant used
Sampling	State by whom the sample was taken (cross out what does not apply) State whether the sample was taken manually or using an automatic sampler State the date the sample was taken Sign the form as the applicant. Sign the form as the sampler
